We provide specialized care for patients with chronic, complex conditions who need ongoing hospital-based nursing and medical care.

What we do

Our Neurological and Medical Complex Care service cares for patients with chronic, complex medical and nursing needs that require hospital-level care.

Patients often have physical disabilities, cognitive impairments or communication challenges, in addition to complex health conditions.

We are able to provide specialized nursing and medical care as well as a wide range of therapies and services to meet the diverse needs of our patients.

Our goal is to help patients improve their quality of life and maintain their current abilities. 

What to expect 

When you arrive at Hennick Bridgepoint, you will meet a number of care team members such as nurses, therapists and your physician. Depending on your specific needs, you may also receive care from other members of our health-care team during your stay.

Our team is committed to the following as part of your care:

  • You and your care partners are actively engaged in your care. In some cases where a patient is unable to make their own decisions, care partners or other decision makers will be included.
  • We will work together with you and your care partners to set realistic goals during your stay. 
  • Depending on your specific needs and reason for admission, you may receive care from different health-care team members such as physiotherapists, occupational therapists and other medical specialists.
  • We will focus on helping you achieve the best quality of life possible. This includes maintaining your current level of abilities, reducing the risk of injury and caring for your health. However, regaining independence is not a primary objective for our complex care patients.
  • We will communicate with you and your care partners about your day-to-day care. Within a few months of your stay, you and your caregivers can also expect a more formal meeting called a care conference. We will talk about your current needs and goals, what to expect moving forward, and answer any questions you may have.

Please note we are a teaching hospital, so you may see medical students, residents and learners from various health professions as part of your care. Learners are supervised by other members of our dedicated health-care team who work together to provide you with comprehensive care. 

Partners in care

You and your caregivers are important partners in your care. Patients and caregivers can speak with any member of your care team about anything related to your care. You can also ask for a family/caregiver meeting with your team to discuss your progress and care at any time. 

Leaving the hospital

We believe every patient has the potential to no longer need hospital-based care. Our complex care patients often stay with us for a long time, but we work together to understand when you may be ready to leave the hospital.

The care team will engage in discussions with patients and/or care partners soon after admission and throughout the hospital stay to discuss discharge options, considerations, goals and timing.
