Geriatric Medicine Consultation Service

The Geriatric Medicine Consultation Service provides assessments and treatment recommendations for older adults who are admitted to Mount Sinai Hospital and Hennick Bridgepoint Hospital with acute medical, surgical and mental health issues. 

Our health-care teams receive specialized training to help older adults recover after a health crisis. We use the ACE approach to support their physical and cognitive health, and we identify the types of support they will need to maintain a high quality of life when they leave the hospital.

What to expect

A member of our health-care team will visit you in your hospital room to discuss your current needs as an older adult in the hospital.  

We will then work with you to decide on a care plan that is specific to your needs. Our goals are to provide the specialized care you need during your hospital stay and help you get back home with the proper support.  

During your hospital stay, your care plan will focus on restoring your health and independence. Your geriatrics team will perform a Comprehensive Geriatric Assessment (CGA). This is a holistic assessment that reviews your medical conditions, mental health, social context and function. They will also touch on common issues experienced by older adults.  

After your hospital stay, your care plan will involve your primary care provider and other community supports. If you need ongoing care after leaving the hospital, your geriatrician may refer you to the Geriatric Medicine Clinics or another service that meets your needs.

A member of your health-care team at Mount Sinai or Hennick Bridgepoint will refer you to our service, if needed.
