Social Work

We support patients with emotional, financial and practical concerns.

Social workers at Sinai Health evaluate patients’ emotional, financial and practical needs. They offer support to patients in hospital and to those receiving care in some of our clinics. 

In Ontario, social work is a regulated profession guided by evidence-based practices. Our social workers also help patients and their families navigate the challenges that often accompany hospitalization and illness, including coping with illness, disability or end-of-life issues.

We help patients throughout their care journey in several ways including:  

  • Offering emotional support and counselling to patients and families  
  • Assisting with transitioning to a new care environment such as rehabilitation or palliative care
  • Acting as a bridge between patients, family and the care team to improve communication
  • Connecting patients with community resources and support networks
  • Providing information about available financial resources such as housing assistance
  • Assisting with discharge planning and ensuring a smooth transition from hospital to home or another care setting
  • Facilitating support groups in specific clinical areas 

How to access our services

Patients and caregivers can contact a social worker through any member of their care team. At Mount Sinai Hospital, patients can also call the Social Work department directly.

Patients and families from out-of-town who need access to discounted accommodations rates while they are in Toronto for care can email us: [email protected].

Additionally, patients from Northern Ontario may qualify for a Northern Health Travel Grant. It is advisable for patients to apply for this grant before travelling to Toronto. Please connect with your referring physician’s office in your home community for additional information.
