You may be referred to any the following physicians for clinic-based appointments to continue your care after you leave the hospital.

Physiatrists

Physiatrists are rehabilitation physicians who treat injuries and illnesses that affect your strength and movement.

A neuropsychiatrist diagnoses and treats mental disorders that are connected to an injury or abnormality in the brain or nervous system.

An endocrinologist is a physician who diagnoses and treats health conditions that are related to hormones.

What to bring

  • Health (OHIP) card or valid health-care coverage (needed for every visit)
  • A list of your current medications
  • A list of your current physicians
  • For diabetes patients, a glucometer and recent lab results
