The ACE approach is a model of care that encourages self-care and independence during and after hospitalization.

What we do

The Acute Care for Elders (ACE) approach focuses on restoring health and independence for older adults while they are cared for in the hospital.

Our health-care teams receive specialized training to help older adults recover after a health crisis. We use the ACE approach to support their physical and cognitive health, and we identify the types of support they will need to maintain a high quality of life when they leave the hospital. 

What to expect  

Your health-care team will work with you to decide on a care plan that is specific to your needs. Our goals are to provide the specialized care you need during your hospital stay and help you get back home with the proper support.  

We will work with you to design a care plan that is specific to your needs, values and preferences. Your care plan may include:

  • Changes to your physical environment to minimize the risk of falls, confusion and stress
  • Discharge planning to help you transition out of the hospital as early as possible
  • Reviewing your medical care as a partner and advocate within the hospital

After your hospital stay, your care plan will involve your family and caregivers, your primary care provider and other community supports. 

If you need ongoing care after leaving the hospital, your geriatrician may refer you to our Geriatric Medicine Clinic or another service that meets your needs.
