Clinical nurse specialist 

Clinical nurse specialists perform assessments and co-ordinate care and support for patients. They also provide other health-care team members with the support and education they need to deliver high-quality care to patients.

Nurses

Geriatric nurses are an essential part of your care team. They administer medications, perform assessments and can give you advice on how to manage your health. 

Social workers

Social workers work with patients to determine their support needs and connect patients with community resources and services to improve their quality of life.

Pharmacists

Pharmacists work with patients to review their medications and make tailored medication plans and adjustments. Pharmacists also assist patients with questions or concerns about their medications.  

Elder life specialist

Elder life specialists coordinate the Maximizing Aging Using Volunteer Engagement (MAUVE) program, which connects specially-trained volunteers with older adults who are in the hospital.
