Orthopaedics Referrals

We accept referrals from primary care providers and specialists. 

Patients with arthritis should refer to the Hip and Knee Rapid Access Clinic. 

If a revision surgery is required, please refer to your surgeon directly.  

We accept referrals from primary care providers and specialists. To help reduce wait times, we process referrals through Ontario Health Toronto. To refer a patient, please select Mount Sinai Hospital on the referral form (PDF) and fax it to 416-586-3213. 

Please fax a completed referral form (PDF) to 416-586-4769. Should you have questions or concerns about a sent referral or patient inquiry, please call 416-586-5041.  

Please note we do not accept referrals for soft tissue or sport medicine related injuries. Please send your referral to the Dovigi Orthopaedic Sports Medicine Clinic. 

Please fax a referral to 416-619-5548 with the following information:

• Patient demographics
• Patient’s OHIP number
• Imaging
• Reason for the referral
• Referring physician’s information
