Referrals are accepted from family physicians and specialists.  

We accept referrals from family physicians and specialists. All referrals should include:

  • Patient's name
  • Date of birth
  • OHIP number
  • Contact information (including mailing address and email address)
  • Reason for the referral
  • Any lab and diagnostic results from tests that have already been completed

Referrals are accepted from physicians and nurse practitioners. Please fax the referral letter to 416-586-8573.

Our clinic is currently experiencing a high volume of referrals due to the specialized nature of clinic and the nation-wide catchment area. We screen referrals for immediately life- or limb-threatening conditions (for example, cancer or torsion).

Patients receive a letter with their appointment time and date through the mail at the mailing address listed on the referral. We send a copy of the appointment date and time to the referring physician.

Patients will be contacted by email.

Please refer patients with prostatitis concerns to fax 416-586-5237. Our office will refer the patient to the prostatitis clinic if appropriate.
