Your personal health records

Your personal health records are confidential documents that include information collected by Sinai Health during your stays and visits at our hospitals and clinics.

This includes both personal information like your name and date of birth and health information like physician notes, test results and your health card number.

As a patient, you have the following rights:

  • Privacy and confidentiality of your personal health information, with limited and specific exceptions
  • Access to your personal health information
  • The ability to request a correction of incomplete or inaccurate information in your health record
  • Access to a copy of your health record upon request
  • Health care providers outside Sinai Health can also request a copy of your health record to continue your care

Accessing your health record

You can see some of your medical test results and lab reports through a free service called MyChart.

See instructions on how to set up your account on MyChart. Note that not all of the information in your health record is available on MyChart. 

You can request a copy of your own health record for a fee. We can send paper or digital copies of your health record by mail, email, fax or they can be picked up in person. You can also consent to having it sent to a third party such as a lawyer or insurance company.

All requests for health records, including birth and death records, should be made by filling out the Consent for Disclosure of Personal Health Information form (PDF) and sending it to the appropriate location below. 

Detailed instructions and information about other documents that you may need to send are on the second page of the form. 

We try to respond to your request within 30 days. 

Please note: The Health Records department does not have records from our outpatient clinics. Please contact the clinic directly to request your record. 

Fees for health records

The base fee for a copy of your health record is $30 for the first 20 pages. There is no fee when another health-care provider requests your health records to continue your care.
