Diabetes Education and Management Program

We offer free diabetes education services to patients and members of the community.

What we do

Our team in the Diabetes Education and Management Program provides free diabetes education and management services to patients and members of the community. We are dedicated to making the process of diabetes management more accessible and effective. 

We welcome patients with type 2 diabetes, prediabetes or those at high risk for type 2 diabetes.  

Our primary goals are to empower people with knowledge about diabetes, help them set realistic goals and guide the management of their health. 

This program is available to patients of physicians at our Vaughan and Toronto sites.

What to expect

During your visit, you will meet with a certified diabetes educator (CDE). A certified diabetes educator is a highly trained nurse or dietitian who helps people with diabetes manage their condition.

The certified diabetes educator can answer questions about using insulin, self-monitoring of blood glucose, medication, preventative care, foot care and meal planning.  

Our team can also help connect you to resources in the community and work with your family physician to develop a personalized diabetes care plan tailored to your specific needs.

What to bring

  • Health (OHIP) card or valid health-care coverage  
  • A list of your current medications  
  • A list of questions you want to ask  
  • A support person, if desired 

How to access our services

If you are a registered patient at the Mount Sinai Academic Family Health Team, please ask your physician to refer you to this program.  

If you would like to become a patient of the Mount Sinai Academic Family Health Team, learn more about becoming a patient.
