We are not currently accepting new patients.

Resident practices

Our Toronto office accepts new patients from time to time into our resident practices.

Please visit this link to apply to become a new patient in our resident practice. Please note this link is only active when we are accepting new patients – if the link does not work please try back in a couple of months.  Alternatively, you can call Health Care Connect at 1-800-445-1822 to connect with a family doctor outside of our practice.

Once you have filled in the new patient form application – our office will contact you with the next steps. Please allow up to 12 weeks to process your application.

Perinatal care

If you are pregnant and do not have a family physician, you can self-refer for maternity care with the Toronto Mount Sinai Academic Family Health Team. To self-refer for maternity care, please fill out a patient registration form Please indicate on the form that you are pregnant and wish to receive care from a family physician for your pregnancy.  

If you are pregnant and you already have a family physician, you must speak with your physician and ask for a referral from your physician to our Maternity Care Team at Mount Sinai Academic Family Health Team.

We accept referrals from health-care providers at the Mount Sinai Academic Family Health Team as well as external physicians.  

To refer a patient, please fill out our referral form (PDF) and fax it to 416-586-3175.  

Note: We do not accept referrals for gestational diabetes, pregnancy counselling or patients on insulin pumps.

To refer a patient for help with insulin titration, please complete the insulin order form (PDF).

We accept referrals from hospitals, primary care providers and LHIN care coordinators. To refer a patient, please complete the HIPS referral form (PDF).

We accept referrals to from family physicians for low-risk prenatal, intrapartum and postpartum care. We do not offer routine maternity care to non-residents of Canada. We advise non-residents to defer their visits to Canada until after their pregnancy has concluded.

After completing the course of care at our clinic, the mother and baby will be returned to the referring physician. Please note maternity care here will not affect Family Health Organization (FHO) services at your location.  

We accept referrals from primary care providers.  

To refer a patient for IUD insertions and removals, contraceptive implant insertion and removal and endometrial biopsies, please complete our referral form (PDF) and fax it to 416-586-3175.
