What we do
Our pharmacist helps the Mount Sinai Academic Family Health Team manage the safe and effective use of medication.
The pharmacist works together with patients and the health-care team to select, monitor and periodically review both prescription and non-prescription medications, herbal supplements and vitamins.
Brown-Bag Medication Check-Up program
Patients with chronic health conditions should book an annual appointment with our pharmacist. It is part of our clinic’s Brown-Bag Medication Check-Up program.
This includes patients with one or more of the following conditions:
- Diabetes
- Heart failure
- Angina/Coronary artery disease
- Hypertension
- Asthma
- Chronic obstructive pulmonary disease (COPD)
During your appointment, the pharmacist will look at your medications and current dosages. Together, you and the pharmacist will check if your medications are effective, safe and still necessary.
The Brown-Bag Medication Check-Up program aims to educate patients to be able to manage their medications safely and reduce preventable hospitalizations. In 2015, this program received the Association of Family Health Teams of Ontario Bright Lights Award for transforming patient and caregivers' experience and health.
This program is only available to patients of physicians at our Toronto site.
What to expect
At your first appointment with our pharmacist, you will be asked about the medication and supplements you currently use. The pharmacist will review your medical history and talk to you about the purpose and potential side effects of each medication and supplement. This is also an opportunity for you to ask questions.
After your appointment, the pharmacist may make suggestions to your health-care team about your medication. You may also have follow-up appointments to review your medication and supplements from time to time.
What to bring
- Health (OHIP) card or valid health-care coverage
- A list of the prescription medicines, non-prescription medicines, herbal supplements and vitamins you use
- This includes: pills, medicated creams, patches, inhalers, sprays, and eyes drops
- A list of questions you want to ask
- A support person, if desired

Patients of the Mount Sinai Academic Family Health Team can book an appointment with the pharmacist at the front desk of the Granovsky Gluskin Family Medicine Centre or by calling 416-586-4800 ext. 5159.
