We are a highly specialized referral centre for complex IBD and other conditions. 

We accept referrals from physicians.  

To refer a patient to a gastroenterologist or colorectal surgeon at Mount Sinai Hospital, please fax a letter with the following information to the appropriate physician’s office, listed below.  

  • Patient's name
  • Contact information
  • OHIP number
  • Relevant medical history  

Please visit our Colorectal Diagnostic Assessment Program (DAP for information on how to refer patients who meet one of the following criteria:

  • A positive fecal immunochemical test (FIT) test
  • Clinical symptoms that need to be investigated in a rapid manner
  • A new diagnosis of colorectal cancer

We accept referrals from physicians and nurse practitioners for patients with IBD who are considering pregnancy or are pregnant for the following reasons:

  • Education and counselling regarding IBD, IBD medications, fertility and pregnancy
  • Concurrent management of IBD throughout pregnancy
  • Assistance liaising with OB/GYN and Maternal Fetal Medicine at Mount Sinai Hospital
  • Second opinion or transfer of care

We accept referrals from physicians and nurse practitioners for patients living at least 100 kilometres from Mount Sinai Hospital for the following reasons:

  • To assist with the diagnosis of IBD
  • To assume ongoing IBD management
  • For a second opinion regarding the treatment and care of IBD
