Promoting Access and Care through Centres of Excellence (PACE)

We are a network that provides virtual appointments to individuals living in rural and remote communities in Ontario. 

What we do

Mount Sinai Hospital is a member of the Promoting Access and Care through Centres of Excellence (PACE) network that is supported by Crohn's and Colitis Canada.  

The PACE IBD virtual care program provides access to quality care for inflammatory bowel disease (IBD) to individuals living in rural and remote communities throughout Ontario.  

Our team of gastroenterologists, colorectal surgeons, nurses and dietitians meet with patients living with Crohn's disease and ulcerative colitis over a secured computer network. We work with local health-care providers to help patients manage their IBD and minimize the need to travel to Toronto for medical appointments.  

Canadian patients outside Ontario can access care through our national pilot program, the Canadian Network for Virtual Access to Specialists for Inflammatory Bowel Disease (CaNVAS-IBD).

What to expect

We will provide you with instructions for logging onto our virtual care platform when we book your appointment. You can use your own computer or mobile device at home for most virtual appointments.

At the time of your appointment you will meet with an IBD specialist from Mount Sinai using a secure computer network. Similar to a regular physician's appointment, a virtual care visit is private and confidential. The appointment happens in real time and is not recorded.  

Your specialist will ask you about your medical history and your symptoms. After this discussion we will work with you to decide on a care plan that is specific to your needs.  

Please note that virtual care is useful for many appointments, but we may not be able to use it for all your visits. For example, if a physical examination by a nurse is needed, your appointment will be in a medical office.  

We may recommend tests to help diagnose and monitor your condition, such as blood work, endoscopic procedures or medical imaging. These tests can often be performed in your own community. You may also need to visit a gastroenterologist closer to your home or, more rarely, travel to Toronto for specialized procedures. 

What to bring

  • Health (OHIP) card or valid health-care coverage
  • A list of your current medications
  • ​The names of specialists you have seen
  • A list of questions you want to ask
