Refer a patient

We accept referrals from family physicians and other health-care providers. 

To refer a patient to a rheumatologist at Mount Sinai, please fax the following information to the appropriate clinic or rheumatologist:

  • Patient's name
  • Date of birth
  • OHIP number
  • Contact information  
  • Reason for the referral  
  • Physician wait times may vary.
  • Please note we can accommodate corticosteroid injections if needed.  

Currently, we do not accept referrals for patients experiencing chronic non-inflammatory rheumatic disorders, such as osteoarthritis, mechanical neck and lower back pain, hypermobility/Ehlers Danlos or fibromyalgia.
