Substance Use During Pregnancy (SUPport) Clinic

We support pregnant patients who use substances or have a history of substance use.

What we do

Our team supports choice in pregnancy options. We provide pregnancy care, counselling, and mental health and addictions care. We care for patients without judgement or stigma and aim to make our clinic a safe space.

What to expect

A team of prenatal care providers, a social worker, and mental health and addictions specialists will be available to provide care for patients.

At your first visit, you will meet with members of our team who will talk to you about your challenges and answer any questions you may have.

It is important to us that you actively participate in decision-making about your care.

Our team will work with you to design a care plan that is specific to your needs.

Your care may include the following:

  • Mental health and addiction support
  • Counselling
  • Regular pregnancy care appointments  
  • Appointments with other specialists
  • Shared care with other prenatal providers
  • Medication
  • Labour and delivery care
  • Postpartum support 

Some patients may continue to see their existing family physician or pregnancy care provider, while others may see us for all of their pregnancy care.

What to bring

You do not need to bring anything to your appointment. If you want to be prepared, the following list can be helpful:

  • Health (OHIP) card or valid health-care coverage (optional)
  • Hospital card (optional)
  • A list of your current medications
  • Reading material
  • A list of questions you want to ask  
  • A support person, if desired

How to access our services

Patient referrals are not required, but are appreciated.

Patients can drop in during our clinic hours without a referral.

Health-care providers can refer a patient by filling out this referral form. Referral forms can be faxed to 416-586-5109.

Patients do not require OHIP coverage or valid photo identification to be seen by our clinic staff. 

Please contact our clinic if you require support with transportation. We can provide transit vouchers for patients who need them.
